Title: Interventional management of supernumerary teeth: an orthodontic and surgical perspective

Abstract: Introduction: Mesiodens it is an extra tooth that most often appears in the upper jaw between the central incisors. Statistically, it occurs more frequently in males than in females. It is one of the developmental anomalies in childhood that can result in a variety of complications, such as malocclusion, food impaction and poor esthetics. Aim: To present a case of mesiodens in an 8-year-old male patient and the course of its multidisciplinary oral surgical and orthodontic treatment, with whom we prevent functional and esthetic orthodontic anomalies. Case report: This case study presented an 8-year-old male patient diagnosed with dens supernumeraria 21. At the first examination, the presence of an atypical, microdontic, fully formed and cranially oriented mesiodens, placed in the middle of the alveolar ridge, was confirmed clinically and orthopantomographically. Oral surgical intervention was carried out. After a horizontal incision from the vestibular side, relaxing distal osteotomy was performed to visualize the mesiodens and expose it. Surgical extraction of the mesiodens was done, an in-toto enucleation of the cystic sac was accomplish and the cavity was filled with Gela tamp. A suture was placed. One month after the surgical intervention surgical field with the primary lateral right incisor also extracted, was in perfect condition. It was recommended extended orthodontic observation. Conclusion: Timely diagnosis and appropriate approach are essential for avoiding potential complications caused by mesiodens, in order to ensure a beautiful smile for a happy and carefree childhood and the development of a mentally and physically healthy individual.
